Overview
Osimertinib is a third-generation, irreversible epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor taken once a day as an oral tablet, sold by AstraZeneca as Tagrisso (泰瑞沙). It is used in non-small cell lung cancer (NSCLC) whose tumours carry an EGFR exon 19 deletion or exon 21 L858R mutation, and in the T790M resistance mutation that appears after earlier EGFR-TKIs such as gefitinib or erlotinib. Current labelling covers first-line metastatic disease, combination use with pemetrexed and platinum chemotherapy, unresectable stage III disease that has not progressed after chemoradiation, and adjuvant treatment after surgery for up to three years. How it works
Osimertinib is an oral targeted pill that blocks the faulty EGFR protein that drives certain lung cancers. It attaches to the mutant protein permanently and switches off the signal that tells cancer cells to keep growing, including in the brain.
Osimertinib is a kinase inhibitor of EGFR that binds irreversibly to mutant forms of EGFR (T790M, L858R and exon 19 deletions) at approximately 9-fold lower concentrations than wild-type EGFR; two circulating active metabolites (AZ7550 and AZ5104, about 10% of parent exposure) share similar inhibitory profiles.

Dosage & administration
80 mg orally once daily, with or without food. Adjuvant treatment: continue until disease recurrence, unacceptable toxicity, or up to 3 years total. Locally advanced stage III, metastatic, and chemotherapy-combination settings: continue until disease progression or unacceptable toxicity. Tablets may be dispersed in 60 mL non-carbonated water for patients who cannot swallow solids, or given via nasogastric tube.
Dose adjustment
No dose adjustment for renal impairment with creatinine clearance 15-89 mL/min; no recommended dose for end-stage renal disease (CLcr <15 mL/min). No dose adjustment for mild to moderate hepatic impairment (Child-Pugh A/B); no recommended dose for severe hepatic impairment. Avoid strong CYP3A4 inducers; if unavoidable, increase osimertinib to 160 mg once daily and resume 80 mg three weeks after the inducer is stopped. Dose reductions (40 mg daily, then 40 mg every other day) or permanent discontinuation apply for adverse reactions including ILD/pneumonitis, QTc prolongation, cardiomyopathy and aplastic anemia. Missed doses are not made up.
