Overview

Dapagliflozin is an oral sodium-glucose co-transporter 2 (SGLT2) inhibitor marketed by AstraZeneca as Forxiga in the EU, UK, Asia and Southeast Asia, Farxiga in the United States, and 安达唐 in China. It was originally developed with Bristol-Myers Squibb. It is approved as an adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes, and since June 2024 also in children aged 10 years and older in the United States. Beyond glucose lowering, the label has expanded to three outcome-based indications: reducing the risk of hospitalisation for heart failure in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors; reducing the risk of cardiovascular death, hospitalisation for heart failure and urgent heart failure visits in adults with heart failure regardless of ejection fraction (2023 US label); and reducing sustained eGFR decline, end-stage kidney disease, cardiovascular death and hospitalisation for heart failure in adults with chronic kidney disease at risk of progression. The agreed dose is 10 mg once daily for the heart failure and kidney indications and for heart failure risk reduction in type 2 diabetes; glycaemic control starts at 5 mg once daily and may be increased to 10 mg. It is not for type 1 diabetes and is not recommended for glucose lowering when eGFR is below 45 mL/min/1.73 m2, although patients already taking it may continue at 10 mg once daily at lower eGFR levels to reduce eGFR decline and cardiovascular events. Dapagliflozin is off patent in many markets. How it works

Dapagliflozin blocks a protein in the kidney called SGLT2, which normally pulls glucose back out of the urine into the blood. With that protein blocked, more glucose leaves the body in the urine, so blood sugar goes down. It works without depending on insulin, and it also makes the kidney pass more salt and water, which reduces the workload on the heart and kidneys.

Reversible, highly selective inhibitor of sodium-glucose co-transporter 2 (SGLT2) in the S1/S2 segments of the renal proximal convoluted tubule. Inhibition reduces renal glucose reabsorption, increases urinary glucose excretion and produces osmotic diuresis, lowering HbA1c independently of insulin secretion or sensitivity. Concomitant natriuresis, reduced glomerular hyperfiltration, lower arterial stiffness and a shift toward ketone-body myocardial substrate use are the proposed mechanisms behind the heart failure and chronic kidney disease benefits.

Dosage & administration

Glycemic control: start 5 mg orally once daily, may be increased to 10 mg once daily for additional glycemic control. All other indications (heart failure, chronic kidney disease, heart-failure risk in type 2 diabetes): 10 mg once daily, at any time of day, with or without food.

Dose adjustment

US label dosing by eGFR: eGFR 45 or greater - 5-10 mg once daily as above; eGFR 25 to less than 45 - 10 mg once daily; eGFR less than 25 - initiation not recommended, but patients already on it may continue 10 mg once daily to reduce eGFR decline, ESKD, CV death and hospitalization for heart failure. Assess renal function and volume status before and during therapy and correct volume depletion first. Mild, moderate or severe hepatic impairment: no dose adjustment in the US label (the UK SmPC recommends a 5 mg starting dose in severe hepatic impairment). No dose adjustment based on age. Not recommended with dialysis.

Side effects

Serious reactions to watch for
  • Diabetic ketoacidosis - rare but can occur with normal or only mildly raised blood glucose
  • Urosepsis and pyelonephritis

Monitoring

  • Renal function (eGFR) before initiation and then as clinically indicated
  • Volume status, blood pressure and symptoms of hypotension, especially in the elderly, renal impairment, low systolic BP, or diuretic use
  • Blood glucose and HbA1c (reassess around 3 months)
  • Ketones and acid-base status if the patient is unwell, dehydrated, fasting, post-surgery, or on a very-low-carbohydrate diet
  • Signs and symptoms of urinary tract and genital mycotic infections
  • Signs of hypoglycemia if on insulin or a sulfonylurea (consider lowering their dose)
