Alopecia areata: 2 mg once daily, increased to 4 mg once daily if the response is inadequate; for patients with nearly complete or complete scalp hair loss, with or without substantial eyelash or eyebrow hair loss, consider treating with 4 mg once daily, and reduce to 2 mg once daily once an adequate response is achieved. COVID-19: 4 mg once daily for up to 14 days. Doses may be dispersed in water for patients who cannot swallow tablets whole. Dose adjustments: Avoid initiation or interrupt treatment in patients with anemia (hemoglobin <8 g/dL), lymphopenia (absolute lymphocyte count <500 cells/mm3) or neutropenia (absolute neutrophil count <1000 cells/mm3) for RA and alopecia areata; for COVID-19 the thresholds are ALC <200 cells/mm3 and ANC <500 cells/mm3. Not recommended in patients with RA or alopecia areata and eGFR <30 mL/min/1.73 m2, and not recommended in patients with severe hepatic impairment for RA or alopecia areata. Dose adjustment is required with strong OAT3 inhibitors such as probenecid.

Elimination half-life: Approximately 12 to 16 hours in patients with rheumatoid arthritis and alopecia areata (total body clearance 8.9 L/h in RA and 11 L/h in alopecia areata). In intubated COVID-19 patients the half-life is 10.8 hours with clearance of 14.2 L/h. Breastfeeding should be avoided during treatment and for 4 days after the last dose (approximately 5 to 6 elimination half-lives).. Dosing intervals are designed around this, which is why most regimens are once or twice daily.

Recommended monitoring for baricitinib: Test for latent tuberculosis before and during therapy (except in COVID-19); treat latent TB prior to use; monitor for active TB; Baseline and periodic lymphocyte count, neutrophil count and hemoglobin; Baseline and periodic liver enzymes; Lipid panel — monitor for increases in lipids; Renal function (eGFR) to check the not-recommended thresholds and dosing; Monitor for serious infections, thrombosis, MACE and gastrointestinal perforation; Monitor for hypersensitivity reactions; Avoid live vaccines during treatment
